25 | On October 5, 2026, Licensing Program Analyst (LPA) Valentin Hernandez conducted an unannounced Case Management inspection regarding an Unusual Incident Report. LPA met with Director of Child Development Edith Cazares. LPA toured the facility and census was taken.
LPA explained to Director of Child Development that the purpose of the inspection was to follow up on a self-reported Unusual Incident that occurred on September 25, 2026, and was reported to CCLD on September 28, 2026 . At approximately 10:30 a.m. on September 25, 2026, during classroom clean-up, Child #2 became upset and threw a toy, which accidentally struck Child #1 on the right temple. Staff members witnessed the incident. The Site Supervisor immediately assessed Child #1, observed redness on the temple, and applied an ice pack. A school accident report was completed and provided to the child’s mother. The Program Director was notified at the end of the day. Child #1 did not require medical attention.
LPA inspected the classroom and observed no blind spots and no issues related to supervision. Based on interviews and records reviewed, this appears to be an isolated incident. Staff responded appropriately, addressed the situation promptly, and followed required reporting procedures.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency cited during today’s visit. Exit interview conducted with Director of Child Development Edith Cazares and a Notice of site visit will be posted for 30 days. Appeal Rights were provided and discussed.
