25 | On 09/03/2026, Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced case management–incident inspection. LPA met with Director Toni Ferguson, census was taken, and both the inside and outside areas of the facility were toured, with specific focus on the outdoor playground area. The purpose of today’s inspection was to follow up on a self-reported unusual incident received by the Regional Office on 07/13/2026.
On 07/13/2026, Child #1 was running on the outdoor play structure during scheduled outside play when the child tripped while descending the steps and hit the area under the nose. Two teachers were supervising 14 children and witnessed the incident. Staff responded immediately, provided first aid, applied an ice pack, and contacted the authorized representative. The child was evaluated at a medical facility and received three sutures.
During today’s inspection, LPA conducted interviews, observations, and reviewed the child’s file, including the parent-signed accident report and medical documentation. Based on the information obtained, LPA determined supervision was appropriate, staff acted within policy, and no hazards on the play structure contributed to the incident. The child returned to care on 07/21/2026 and was observed in care today.
An exit interview was conducted, and the report was reviewed with Director Toni Ferguson. Appeal rights were provided.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. A Notice of Site Visit was provided and must be posted for 30 days.
