On 08/06/2026, Licensing Program Analysts (LPAs) Denisia Jimenez and Aurelio Mendoza arrived at the facility to deliver investigation findings to the allegations. LPA met with Regional Director Amy Miller and a census was taken.
LPA interviewed the reporting party, classroom staff, and the director. Interviews and records indicated child sustained minor incidents, including a push, a scratch, and a prior bite that occurred during typical toddler interactions in a supervised classroom. Staff reported providing appropriate care, redirection, responding immediately, and parent notification.
Staff and Director stated diaper checks were completed on schedule, with additional changes as needed. One communication lapse regarding a partial bowel movement was acknowledged and corrected. No evidence indicated the child's diapering needs were neglected.

No witnesses reported inadequate supervision or neglectful care. Based on interviews and available documentation, there is insufficient evidence to support that staff neglect caused the injuries or failed to meet the child's diapering needs.

Based on the information obtained and evidence reviewed, there is insufficient evidence to establish that staff neglect resulted in multiple injuries or that staff failed to meet the child’s diapering needs. Both allegations are UNSUBSTANTIATED.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations no deficiencies are cited.
An exit interview was conducted with Regional Director Amy Miller. A copy of this report and Appeal Rights were provided. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
