25 | On August 13, 2026, at 10:49 AM., Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office. LPA met with Michelle Satorhelyi, the facility director who granted access. At 10:55 AM., LPA and the director toured the facility and observed no day care children in care and no staff. The school will reopen August 17, 2026.
On July 31, 2026, the (LIC 624) reported information to the region office stated, on July 30, 2026, during outside play at 10:00 Am., C1 was going down the slide and at the end of the slide, climbed over the side instead of exiting the slide in the open space in front of them. C1 fell and landed awkwardly on their left arm. Staff gave C1 an ice pack and the parents were notified.
During this inspection, LPA conducted interviews with relevant parties involved and reviewed records. At 10:55 AM., LPA conducted a safety inspection of the playground and observed a large play structure with four straight slides, one curved slide, climbing wall, and bridge with rubber chips underneath and a sign that stated, “for kids two to five years old.”
Based on interviews and record reviews there were two staff near the play structure, one near the slide ten feet away and one on the opposite side of the play structure when C1 fell. C1 notified the staff of their injury and staff provided aid. C1 was able to demonstrate to the staff and parents when they were sliding down, they exited the slide on the curved side and not the front landing of the slide and fell and landed on their arm. Therefore, this incident was deemed as an accident. Staff notified Community Care Licensing (CCL) of the incident.
