32 | Throughout the course of the investigation, LPA Lipsey obtained copies of children’s roster, child’s preadmission health history, the physician’s report, took photos, and conducted interviews with one parent and seven staff members.
Pertaining to the allegation, “Facility does not have qualified staff to meet the needs of the child.”
Per the Reporting Party (RP), staff are not trained to meet the child’s needs due to the child’s communication challenges. Per staff interviews, they communicate with the child using gestures, a communication board with photos of items the child may want, and a visual calendar with photos representing parts of the day such as nap time, play time, or lunch time, to help the child understand transitions. Per Staff #1 (S1), an adult is assigned to stay with the child to help prevent injuries. Per Parent #1 (P1), they also use gestures to communicate with the child at home without any formal training.
Pertaining to the allegation, “Staff does not ensure child's records are properly maintained.”
Per the Reporting Party, the emergency files in the emergency backpack contain outdated information, including records for children who no longer attend the center. LPA observed that the emergency backpack contained the LIC 700 emergency forms with current parent contact information for the children present. Although the backpack included names of children not currently in attendance, the information for enrolled children was up to date. The department does not require the center to have contact information in the backpack, however the forms in the children's files should to be current. LPA observed the files located in the office to be up to date and current.
