32 | During interviews, all three staff working in the classroom on 8/27/26 stated they did not change C1's pull-up while in care for the 5 hours and 56 minutes, and C1’s pull-up was not changed at all on that day until C1 was picked up and it was brought to the attention of Staff #2, by the person picking up C1, that the pull-up needed to be changed. Staff #2 stated they changed the pull-up at that time and observed the pull-up was wet and C1 had had a bowel movement.
Director stated, “I take ownership for this, and it won't happen again”. All three staff interviewed stated the facility did not keep a diaper log on 8/27/26, but the facility has started to keep a diaper log since that day in an effort to ensure children's diapers and pull-ups are changed in a timely manner.
Based on interviews conducted and records reviewed, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC 9099D.
