On 9/23/26 (LPA) Licensing Program Analyst Kamile Martin conducted an unannounced annual inspection. Upon arrival licensing staff met Lead teacher Alicia Banda. LPA went on a self guided tour of the facility. Per teacher, there are currently 9 enrolled in the Morning class and 7 enrolled in the PM classroom. Hours of operation are Monday-Friday 7:30am-10:00am Morning Session, and the PM session is 11:45am-2:45pm. The program operates at Vejar elementary school in Rooms 47, 33, and 34. Entrance to the State Preschool program is from parking lot off of White Ave. Room 47 is directly in front of playground next to the parking lot. Rooms 33 and 34 entrance gate is near the school entrance of the parking lot. Classrooms 33 and 34 are a full day program and the operation hours are 7:30am-2:00pm. Per teacher Vijeta Bagga children in room 33 have early dismissal at 11:00 am due to extreme rise in heat and hot temperatures in the classroom. LPA observed two large portable AC units and two fans operating in the classroom.

All areas identified on the Facility Sketch were inspected. The facility consists of 3 classrooms and 2 outdoor play areas. The following was observed: Room 33 and 34 are Head Start Preschool classrooms and are a full day program, There were 11 Children and 3 staff observed in Room 33 with 12 enrolled, and Room 34 had 11 children and 3 staff, with 13 enrolled. Room 47 is a State Preschool program with an AM session and a PM session, which had 6 children and 2 staff with 9 enrolled in the morning and 7 enrolled in the afternoon session. Teacher child ratios were observed to be in accordance with Title 22 regulations. All children were observed to be under active/visual supervision of a teacher during the tour.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Licensing staff observed disinfectants, cleaning solutions, medication and other items that are dangerous to children were not accessible to children. Classroom furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All floors are clean and safe. Napping equipment (mats/cots) and bedding were inspected for good condition, appropriate storage, and cleanliness. Children's linens are washed weekly by a washing service. All toilets and washing facilities are in safe and sanitary operating conditions. First Aid supplies are available and complete. Drinking water was readily available indoors and outdoors. Children who fall ill are away from other children near the entrance of the classroom which is used as an isolation area there is a chair available for an ill child to rest and the restroom will be used if needed.

LPA entered the classroom and observed age-appropriate fixtures, furniture, equipment and supplies. Toys and play equipment were also observed to be appropriately clean and safe. All surfaces and materials accessible to children, including toys, are toxic free. LPA observed the facility has portable AC unit observed and operating in classroom Room 47, and Classroom 33 has central AC, While classroom 34 was observed to have two operating fans an two large portable AC units. Overhead lights as well as natural light was observed in all classrooms. LPA observed the classrooms have cubbies to store children’s personal belongings. Licensing analyst observed trash bins with tight fitting lids in the classrooms. LPA observed the restrooms used by children to be in operable condition, supplied with toilet paper, Girls and boys share one restroom and there are two toilets separated by a divider in classroom. There are two sinks inside of the restroom and one outside of the restroom for children’s use after outdoor play time. LPA observed paper towels and hand soap were provided near the sink area. All areas were inspected for safety and sanitation. Preschool age children nap in classroom 33 and 34. Napping mats are stored on the storage rack located at the back of the classroom. Laundry services come weekly to pick up dirty bedding and provide clean bedding.

The facility provides filtered water for children’s use with a filtered water jug. Each child has a cup with their name labeled on the cup. The cups are stored separately and are taken out on a cart for children to use during outdoor time for children to drink freely. The elementary school provides Breakfast and Lunch. PM Snacks are provided by the nutrition department.

The Campus Supervisor brings meals to the classroom during mealtimes. Menu items were observed posted on the refrigerator. All meals are prepared and provided by the facility staff. The kitchen area was observed to be free of rodents and vermin. Cutting knives were inaccessible and cleaning solutions were inaccessible to children. All food stored follows expiration dates. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents and any other vermin and storage containers for solid waste including movable bins with tight fitting covers are kept on. The facility was observed to be free of flies, other insects and rodents. All food has expiration dates labeled, canned and dry foods were stored appropriately. Refrigerator temperature logs and meal count logs were accurately recorded. There is a fully equipped first aid kit available in each classroom.

Smoke detectors and carbon monoxide were observed and not tested due to being hardwired. Fire extinguisher type 2A-10BC (inspection date 9/12/26) must be serviced annually.

Fire and earthquake drills must be conducted every 6 months and documented. Emergency drill conducted on 9/21/26. Lock-down Drill was conducted during LPA’s Inspection in classroom 34. Lock-down drill began at 10:32 and ended at 10:40 am. Hazardous items including disinfectants, poisonous, cleaning solutions and any other items that could pose a danger to children were observed to be stored inaccessible to children.

Licensing staff went on a self guided tour to the outdoor play area. LPA observed age-appropriate outdoor toys, play equipment and materials to be clean, and safe for children’s use. There was adequate shade available by use of a shade tarp. All areas around play equipment and slides are cushioned with material to absorb a fall. No bodies of water were observed on the premise. Playground toys were observed outside for children’s use, LPA observed a soft mat to sit on, large manipulative, tricycles, various size balls, a sand table, and an art easel, books and manipulative.

Children’s records, roster, and staff files were not reviewed at this site inspection and will be reviewed at a later date due to files stored at Pomona Unified School District. Citations for deficiencies are not being cited during this visit.

Teacher was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance with the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. Per maintenance and facilities manager Kym Allen confirmed water lead was tested 4/21/26 a list of sites was emailed to LPA Kmartin results are pending. LPA will follow up with facilities and maintenance for final the report.

LPA referred Teacher staff to the Department website for lead: Lead Toxicity Prevention and Water Testing Information (https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information)

LPA discussed the safe sleep regulations with site supervisor and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA reviewed safe sleep documentation log for two infants in care. No infants or toddlers are in care.
