On 08/10/2026 Licensing Program Analysts (LPA) Cristina Castellanos and Judy Laureano made an unannounced visit to the above-mentioned facility for the purpose of delivering complaint findings. LPAs met with Director Erin Tamashiro and discussed the purpose of the visit. LPAs toured the facility and observed 20 children in care with 3 staff members providing care and supervision.

On 05/28/2026 Licensing Program Analyst s(LPA) Cristina Castellanos and Brittany Lovest arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations (initial visit). During the course of the investigation LPA requested and reviewed the following documents: Children's Roster (T2a & TK1b), Children’s Files and Academy Parent Handbook. Additionally, that inspection LPAs initiated staff interviews and documented observations.

Based on observations, interviews of all relevant parties and record review, no information revealed that staff did not follow admissions agreement and staff did not adequately supervise children. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted with Director Erin Tamashiro. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
