On 1/12/2023 Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced subsequent investigation to deliver complaint findings for the above allegations. LPA toured the facility and observed 5 children in care with the licensee providing care and supervision.
The investigation was conducted by Investigations Branch(IB), Investigator Christine Ferris. The investigation consisted of interviews with licensee, children and other parties. Statements, and other relevant documentation obtained during the investigation did not revealed day care child sustained unexplained injury requiring due to lack of supervision. Interviews conducted and other documentation did not disclose licensee did not notify day care's representative of injury in a timely manner. These allegations are determined based on interviews and other relavant supporting documentation obtained during this investigation. The above allegations are deemed Unsubstantiated, although the allegations may have happened or are valid, at the time of the investigation there is not a preponderance of the evidence to prove that the alleged violations occurred.
An exit interview was conducted, appeal rights notice of site visit, and a copy of this report was provided to licensee on this date.
