32 | For each incident, the parent was notified of the injury and signed the Accident/Incident report acknowledging the injury occurred. Based on the regulation 101226 (a)(2) in the case of less serious injuries including, but not limited to, minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the nature of the injury when the child is picked up from the center. Therefore, the allegation does not corroborate with the evidence. Allegation #2- On May 19, 2026, the facility provided a written letter of consistent behavior observed by (C1) in their classroom. On June 15, 2026, there was a meeting with the parent to discuss a plan partnering together as a team for school and home support. During that meeting, it was discussed and agreed that an outside therapist would come and assist, there would be a change in scheduled pick-up time to 3:00 PM., the facility will provide one-on-one staff assistance for five weeks, and there would be another meeting to discuss progress. From May 27, 2026, to August 4, 2026, there were eighteen therapy sessions that occurred from an outside therapist. On July 29, 2026, there was a request from the facility for a meeting with parents for a progress update to where the meeting kept being changed by the parent. On August 7, 2026, the parent requested to pick up the child at 5:00 PM. The meeting occurred on August 12, 2026, and instead of a progress update, a termination letter was given to the parent, with the acknowledged receipt date dated July 12, 2026. Staff stated that the letter was dated by the parent with the wrong date. The facilities Admissions Agreement states, A child may be terminated from the school if it is deemed to be in the best interest of the child or if the school’s policies and procedures are violated. It is determined the facility and parent initially partnered together to support the needs of the child, but due to scheduling conflicts, there was inconsistency with the parent being able to accommodate the agreed pick-up schedule which resulted in the facility not being able to provide staff support to assist the child. Therefore, the allegation does not corroborate with the evidence. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore both allegations are Unsubstantiated.
An exit interview was conducted, a copy of this report was read, and a Notice of Site Visit, was given to Soumya Shetty, at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain the posting may result in $100 civil penalty.
