Based on the interviews conducted, and reports reviewed, there was insufficient evidence to either support or confirm the above allegations. This agency has investigated the complaint alleging that staff were falsifying incident reports, and that staff had wrongfully terminated a child from the program. At this time, it is determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegations are unsubstantiated.
