On 07/09/2026, at approximately 7:30am Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced compliant inspection at the facility noted above. LPA met with facility representative 2 (F2) and explained the purpose of today’s visit. There was one child and F2 when visit began.

At approximately 8am, Staff 1 (S1) arrived to the facility.

At approximately 12:00pm, Staff 2 (S2) arrived to the facility

The purpose of the visit is to deliver findings for three complaint allegations: Uncleared Adults in the facility, Owner is not qualified and is caring for children, and facility operates over ratio. During this investigation, multiple visits were conducted to make observations, collect documents, and conduct interviews. No verified evidence was obtained therefore allegations are unsubstantiated.

LPA made observations, conducted interviews, and reviewed visitor sign in and out sheets. It was reported that an adult that was not fingerprint cleared comes to the facility. During visits to the facility,

LPA J. Culbert did not observe children interacting with anyone other than staff who are fingerprint cleared and parents. LPA reviewed the visitor sign in and out sheets and based on F2's explanation for each individual listed, a fingerprint clearance was determined unnecessary. LPA interviewed witnesses and the information obtained did not support the allegation.

Owner is not Qualified and is Caring for Children

LPA interviewed and reviewed staff files and conducted interviews. During staff review, LPA observed that all staff files were complete. When asked who provides care to the children at the facility, all staff members gave the same response. LPA J. Culbert verified the files of all names provided. The information obtained did not support the allegation.

Facility Operates over Ratio

During this investigation, LPA J. Culbert conducted seven visits and observed children to staff ratios. The greatest number of children LPA observed at one time were twelve children with three staff. The greatest number of children enrolled did not exceed 17. Today, there are nine children and two staff. LPA did not observe the facility out of ratio at the time of inspections. LPA reviewed staff timecards from April 2026 to June 2026 and compared it with information on the children's Monthly Attendance Summary. The number of children did not exceed thirteen and on the days when thirteen children were present at least two staff were available. During interviews, the majority agreed that there were no more than twelve children present at the facility at one time. LPA observed this to be true for the month of June. Based on the observation, documentation, and interviews the allegation that the facility operates over ratio is found to be unsubstantiated.

At approximately 1pm, F2 left the facility.

Based upon evidence obtained during this investigation, the allegations that, " uncleared adults are in the facility, owner is not qualified and is caring for children, and the facility operates over ratio," have been determined to be unsubstantiated. The allegation may have happened or is valid, but there is not a preponderance of evidence to prove that the alleged violations occurred.

No deficiencies were cited during today's visit.

Exit interview conducted and report was reviewed with S1. Notice of site visit was given and must remain posted for 30 days.
