25 | Licensing Program Analyst (LPA) Rita Ramos attempted to conduct an unannounced required 1 year inspection to the above facility on 11/23/2021. LPA arrived at the facility at 1:20 PM, identified self and met with Trevonne Napier, Licensee, who at arrival informed LPA that they have not had any children in care since July of 2021. Licensee had their own child present during the visit.
During the visit, Licensee and LPA discussed Licensee's option on whether or not they would like to go Inactive or surrender their license. LPA advised Licensee that if they wish to go inactive they would have to comply with keeping their Pediatric First Aid & CPR card up to date as well as their fire extinguisher, have operable carbon monoxide detector, smoke detector, and have their annual fees current.
Upon discussing Licensee's options, Licensee decided to surrender their license during the visit and provided the original license to LPA during the visit along with a written and signed declaration. Due to Licensee expressing an interest to re-apply in the future, LPA advised Licensee to keep up to date on Title 22 regulations via the department's website: www.ccld.ca.gov.
.
The facility will be closed out effective today's date.
Exit interview conducted and report was reviewed with the licensee, Trevonne Napier. .
