Interviews were conducted and it was stated yes the incident occurred as the child was lifted by the hand mistakenly by the staff member and captured on video. The staff member was very apologetic and understood her mistake. Parent has been spoken with by the staff member and the assistant director. This was an isolated incident by the staff member and self reported by the facility.

LPA observed the video and it clearly shows the shadow of a child being pulled by the hand/wrist and slightly lifted by the hand/wrist by an adult female.

Based on interviews, video footage and disclosures made during the department investigation, the preponderance of evidence and the standard has been met, therefore, the above allegation is found to be substantiated. California Code of Regulations (Title 22 Division & Chapter), are being cited on the attached deficiencies page LIC 9099-D. Consultation was given on this date.

Exit interview conducted and report was reviewed and given to Jessica Murphy, Director.
A notice of the site visit was given and must remain posted for 30 days.
