32 | unable to remember what days or month A1 assisted with children pick ups. Individuals interviewed state that either the licensee, assistant, or their own family member would pick children after school and bring them to the facility. During the inspection, LPA observed that the outdoor play area had a garden hose unraveled throughout the ground of the play area. Sections of the back yard play are covered in an green artificial grass mat. LPAs observed holes in the mat located on the ground. The wooden playhouse was observed to be worn down with exposed splinters on the side. Plastic dramatic play sets were observed toppled over in the pathway in the back yard play area. Based on LPAs observations, and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. LPA advised licensee that a citation under California Code of Regulation (CCR) section 102417(g) and Health and Safety Code Section 1596.871(c)(1)(A) will be issued.
LPA R. Derraco informed licensee Karina Hernandez that this report dated 06/11/26 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
