25 | On 10/01/2026, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced case management inspection at the facility noted above. LPA met with facility representative, Maria Milliken and stated the purpose of today's visit. The purpose of the visit is to follow up on an incident that occurred on 08/14/2026.
On 08/14/2026, Child 1 (C1) experienced an allergic reaction after consuming food provided by staff. The facility representative reported the incident to the Department on 08/28/2026, which is 14 days after the incident occurred. This delay constitutes a potential health, safety, or personal rights risk to persons in care.
On 09/11/2026, LPA reviewed C1's file and interviewed the facility representative. During the file review, LPA observed documentation indicating that C1 is allergic to egg. The facility representative confirmed that staff were aware of C1's food allergy: however, they did not realize that the sandwich provided to C1 at the potluck contained mayonnaise, which includes egg. This is an immediate health, safety, or personal rights risk to persons in care.
LPA Jonnisha Culbert informed facility representative, Maria Miliken that this report dated 10/01/2026 documents one Type B and one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Jonnisha Culbert informed the facility representative, Maria Miliken, to provide a copy of this licensing report dated 10/01/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report.
