25 | On April 5, 2024, Licensing Program Analyst (LPA) Garcia, conducted an unannounced plan of correction (POC) visit to facility. LPA met with licensee Patricia Abreu and explained the purpose of the visit.
On February 28, 2024, LPA met with licensee to conduct an unannounced, annual inspection at facility. During annual inspection, Licensee was cited for not having immunization records for her employee and over capacity, for having 5 infants in care at time of visit. The planned POC due date was 3/28/24. Per licensee, she sent an email of the corrections but LPA never received it. LPA verified to see if there are any records of sent messages from licensee but unable to retrieve them.
As of this date, LPA observed and reviewed records of the employee's immunization records and facility roster. The immunization records for TB, flu declination form, Tdap and MMR for employee are on file and there were 4 children under the age of 2 and 1 child over the age of 2 in care. LPA confirmed with Licensee the correct LPA email address and to call or text the LPA to notify if she runs into other technical issues. During the visit, Licensee sent multiple test emails to LPA and they were unable to be received.
Deficiency cited on March 28, 2024 has been cleared. LPA provided facility representative copy of POC letter.
A notice of site visit was given and must remained posted for 30 days.
Exit interview conducted and report was reviewed by licensee, Patricia Abreu. |
