On 01/23/2024, LPA attempted to interview 7 parents, however only 2 parents were available for interview. Parents interviewed stated they were satisfied with the day care and were notified of any outbreak that occurred at the facility. Parents interviewed also stated being unable to enter the facility.
Based on interviews with 3 staff members, facility inspection conducted, and interviews with 2 parents, it has been determined (1) Licensee did not ensure that knife was stored locked and inaccessible to daycare children, (2) Licensee did not report a communicable disease outbreak to appropriate parties, (3) Licensee did not allow a parent to enter the facility. Therefore, the preponderance of evidence standard has been met, therefore the above allegations are found to be Substantiated. California Code of Regulations, Title 22, 1012423 (a)(2) Personal Rights, 102416.2 (c)(3) Reporting Requirements, and 102419 (a)(1) Admission Procedures and Parental and Authorized Representative's Rights are being cited on the attached LIC 9099D.
LPA Lucero informed Licensee Svitlana Sidyenikova that this report dated 02/01/2024 document(s) one (1) Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
