25 | Licensing Program Analyst (LPA) Carmen Odom conducted an unannounced case management inspection regarding a self reported unusual incident that was reported to the department on 09/25/21. Upon arrival LPA Odom met with Licensee Gloria Castillo. At 3:40pm, LPA Odom observed a total of 2 preschool age children and 1 school age children present in the day care area playing with one assistant, Paula Perdomo. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 09/25/21 the facility self-reported that on 09/22/2021 at 4:36pm Child #3 (C3) was crying and not wanting to eat, asking for mom. Licensee (S1) informed Parent#1 (P1) via text message that C3 was crying and not wanting to eat. P1 asked S1 to speak with C3 over the phone, S1 called P1 via Facetime and C3 spoke with P1 via facetime. During the phone conversation with P1 while at the childcare C3 told P1 that S1 is hitting C3 while in care.
During the course of the investigation, LPA reviewed incident report, children’s roster, interviewed 3 staff members, 2 children, and 3 parents. At the time of the incident there were 2 witnesses present when C3 told P1 that S1 was hitting C3 while in care. Child #1 (C1) stated, the last day C3 was at the childcare C3 was on the phone talking to P1 and C3 told P1 that S1 was hitting C3, but that never happened C3 lied. C1 stated, C3 lied because he wanted to go home with mom. Child #2 (C2) stated, C3 told P1 that S1 spanked him, but that was a lie, don’t know why C3 lied. Both children disclosed S1 and assistants don’t yell or hit the children in care. LPA Odom attempted to interview four parents, but only three parents were interviewed, all the parents disclosed that they are satisfied with the childcare and do not have any concerns.
