32 | On 6/12/26, the Orange County Child Care Office received a complaint alleging (1) Staff did not report injuries to child's authorized representative and (2) Staff do not provide adequate supervision, resulting in child sustaining injuries. The Reporting Party (RP) stated the following: Child#1 (C1) has sustained multiple injuries while in care and believes there is a lack of supervision at the center. C1 has scratches and bruises that were not reported and the school could not provide any explanation. Another child threw something and hit C1 in the face causing an injury.
During the investigation, the LPA interviewed staff members, and parents, and obtained pertinent documents.
Regarding allegation (1) Staff did not report injuries to child's authorized representative
During the investigation, the LPA interviewed five staff members. All five staff members stated that they provide parents with an incident report when their child is involved in an incident. Staff #5 (S5) stated that incident reports were provided to C1’s parents when C1 was involved in incidents. The facility provided the LPA with copies of the incident reports that were given to C1’s parents.
Regarding allegation (2) Staff do not provide adequate supervision, resulting in child sustaining injuries
During the investigation, the LPA interviewed five staff members. All five staff members stated that they provide supervision for the children and did not disclose any concerns regarding their supervision to the children. Staff #3 (S3) stated that S3 was in the classroom carpet area while C1 and Child #2 (C2) were playing. C2 had hit C1 with a toy. S3 stated that S3 attempted to intervene before the incident occurred by walking up to C1 and telling C1 to tell C2, “No, thank you.” S3 stated that initially, there were no visible marks on C1’s face. S3 attended C1 and ensured that C1 was okay. S3 provided C1 with an ice pack, at which redness began to appear on C1’s face. C1’s parents arrived and picked up C1 and were notified of the incident. LPA attempted to interview C1 but C1 did not qualify to be interviewed due to C1’s young age and development level. LPA attempted to interview 2 more children, but the 2 children did not qualify to be interviewed due to their young age and developmental level.
