25 | On 08/23/2023 at 11:40 AM, Licensing Program Analysts (LPAs) Raymond Moorehead and Elyse Jones arrived at the facility in order to conduct a required annual inspection. Licensee stated that she currently did not have any children enrolled or in care. Upon arrival the Licensee stated the facility was not in compliance to reopen and that she would need some time to get back into compliance. LPAs informed the Licensee of her options. Licensee agreed to have the facility go on inactive status until the facility is in compliance and she is ready to care for children. LPAs provided the licensing form LIC 9211 Request for Inactive Child Care License Status which was filled out by the Licensee and returned to the LPA during the inspection.
LPAs discussed the following terms and conditions that come with an inactive status:
- Fees must still be paid timely while on inactive status
- Licensee can only provide care for the children of one family or blood related
Prior to activating the license, the Licensee must notify the Department, and a comprehensive visit will be conducted to verify that the facility is in compliance. Once the facility is in compliance with Title 22 Regulations, the license will be placed on active status.
Per Licensee's request this facility is placed on inactive status until 09/30/2023.
A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with Grace Duarte, Licensee.
