On 6/28/2022 at Licensing Program Analyst (LPA) Susan Brewer, arrived at the facility unannounced for the purpose of clearing a pending Plan of Correction issued on 06/16/2022 during an annual inspection. LPA was greeted by Licensee Gloria Alipio, and granted entry into the home. LPA conducted a COVID-19 pre-screening and no concerns at this time. LPA took a census of 8 children in care, where 6 children in care were under the age of 2 and supervised by the Licensee Gloria Alipio and Staff 1.

The following POCs were cleared for the following deficiencies during this inspection:
1. 1596.8662(b)(1) Health and Safety Code - A licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. LPA Verified the violation issued as corrected on 06/28/2022.

The following POCs were NOT cleared for the following deficiencies during this inspection for the violations issued on 06/16/2022.

LPA S.Brewer conducted a review of children's records which revealed that the Infant Child #15, on the Confidential Names list, was present during the inspection and the Licensee's Gloria Alipio and Zaida Gloria failed to maintain all required Licensing documents for enrolled children, therefore the violation was NOT CORRECTED ON 06/28/2022.

2. 102417(g)(7) Operation of a Family Child Care Home - An emergency information card shall be maintained for each child and shall include the child's full name, telephone number and location of a parent or other responsible adult to be contacted in an emergency, the name and telephone number of the child's physician and the parent's authorization for the licensee or registrant to consent to emergency medical care.

On 06/28/2022 LPA S.Brewer conducted census and observed 6 of 8 children present and in care were Infants referenced on the Confidential Names list as Child #1, #2, #3, # 6, #11 and #15 are under the age 2 years old and the licensee Zaida Gloria admitted to not correcting the violation issued on 06/16/2022 and knowingly continued to operate over the licensed ratio and capacity. The violation was NOT CORRECTED ON 06/17/2022, as agreed upon,
3. 102416.5 (d) Staffing Ratio and Capacity - For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home and the assistant provider's children under age 10, shall be either: (1) Twelve children, no more than four of whom may be infants; or
(2) More than twelve and up to fourteen children only if the criteria in Section 1597.465 of the Health and Safety Code are met.

LPA Susan Brewer, informed licensee Zaida Gloria and Gloria Alipio, that this report dated 06/28/2022 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

A Civil Penalty has been assessed during this inspection 06/28/2022. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.

Also, LPA Susan Brewer, informed the licensee Zaida Gloria and Gloria Alipio, to provide a copy of this licensing report dated 06/28/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

LPA S.Brewer, conducted an exit interview to review the report. A copy of this report was provided to Licensee Zaida Gloria and Gloria Alipio and must be available upon request for public review for 3 years.


Appeal rights discussed and a copy of this report was provided to the licensee on this date.

The licensees Zaida Gloria and Gloria Alipio, agree to maintain records for every enrolled child in full-time or part-time care, prior attending the daycare facility and will submit a statement of understanding to the departmenty by mail, e-mail or fax on or before 06/29/2022.

Based on LPAs review of records of child #15, present in care, the Licensee Zaida Gloria, admittedly failed to maintain required Licensing documents, LIC700, LIC995A, LIC627, LIC9150, LIC9224, Infant Sleep logs and immunzation records, which is a potential risk to the health & safety of children in care.

LPA conducted a census of 8 children in care of the Licensee's Zaida Gloria & Gloria Alipio, where 6 of 8 present were infants under the age of 2 yrs old & the licensee failed to correct a violation issued on 06/16/2022 and due on 06/17/2022, which is an immediate risk to the health & safety of children in care.
