13 | On 09/09/2026, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation. LPA arrived at the Center and observed 37 preschool aged children being supervised by 3 staff members during nap time. LPA accessed Guardian to determine that all required adults were background cleared and associated to the license. LPA met with Director Donyale Roberts (D1). Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that staff members left a child in a soiled diaper for an extended period of time.
Interviews and documentation revealed that on 09/01/2026, the 2 year old classroom had 1 teacher supervising children and changing diapers. Diaper logs revealed that diaper changes were not conducted every 2 hours.
Based on interviews, file reviews, and observations conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-A deficiency was cited on a subsequent 9099-D page.
