13 | THIS IS AN AMENDED REPORT*
On June 21, 2022 at 1:55 pm Licensing Program Analyst Lea Habtom met with designated staff Valyncia Nims to amend the complaint status from confidential to public. The census for today's visit was 17 napping children being supervised by 1 staff.
Staff did not report incidents to authorized representative
During the investigation, LPA Habtom toured the facility, conducted observation and interviewed those pertinent to the investigation. It was alleged that staff did not report incidents to authorized representative. Interviews with the reporting parting and witnesses revealed conflicting information regarding notifying authorized representaitive of injuries while in care. An app, procare, is used to document injuries and communicate with authorized representatives. A few incidents were shown to LPA regarding injuries. Although the allegation staff did not report incidents to authorized representative may or may have not happened, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore, the allegations are UNSUBSTANTIATED.
