32 | LPA determined staff have established bottle-handling procedures, which corroborate with LPA’s record review of child daily reports. Further, based on confidential interviews children are changed and cleaned as needed while in care. LPA did not gather supporting evidence to support the allegations. Based on LPA's observations, interviews, and record reviews the preponderance of evidence standard has not been met, therefore the above allegations are found to be UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations.
An exit interview was conducted, the report was read, and a copy of this report was left with Director with notice of the site visit and appeal rights. Failure to maintain posting of the Notice of Site Visit for thirty (30) consecutive days will result in a $100 Civil Penalty.
