32 | It was alleged that the facility representative did not ensure that reporting requirements were followed as a result of an outbreak.
A review of the documentation found that an Unusual Incident Report (UIR) (LIC 624) was submitted to the Department on 06/25/2026. However, there were subsequent cases associated with the outbreak, with the initial case occurring approximately nine days prior to the submission of the UIR.
According to Title 22, Section 101212(d), “Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.”
Facility representatives shared that they had reported the outbreak case to the local Health Department and were advised of reporting procedures that differed from what they had typically been doing. Although the center subsequently submitted an Unusual Incident Report (UIR) to the Department, the report was submitted approximately nine days after the initial case. In addition, no documentation of a telephone call or fax from the center to the Department was available for review.
Based on documentation received and reviewed, the department has determined the preponderance of evidence standard had been met, therefore the above allegation is found to be SUBSTANTIATED, according to the California Code of Regulations, Title 22, Division 12.
See LIC809D for deficiencies cited.
An exit interview was conducted with the facility representative, Catherine Skinner. Appeal rights were discussed and provided, a copy of this report was issued, and a Notice of Site (NOS) Visit was also provided.
