there is insufficient evidence to corroborate if the Licensee yelled at Child 1 while in care. For Allegation #5, based on interviews and observation, there is medication inside the home that is made inaccessible to children. LPA did not observe Melatonin therefore, based on interviews, there is insufficient evidence to corroborate Child 1 received Melatonin from the Facility Representative while in care. Therefore, Allegations #1-4 for Personal Rights and Allegation #5 for Medication are Unsubstantiated, meaning the allegations may have happened or are valid, but there is not a preponderance of the evidence to prove the alleged violations occurred.

A copy of this report was delivered to residence of Heather Yarber and placed on the door at the facility and a photo was taken.
