On 08/25/2026 at 8:30 AM, Licensing Program Analyst (LPA) Annette Sutherland arrived at the facility to conduct a complaint inspection regarding a complaint received on 08/20/2026. During the investigation, LPA interviewed multiple parties and reviewed relevant documentation pertaining to the allegations. LPA met with Facility Owner/Director Alexis Velasquez.

Based on interviews conducted and records reviewed, the investigation determined that facility staff administered prescription medication to a child (C1) in care prior to obtaining the required written authorization. Information obtained during the investigation indicated that the medication was provided to the facility on 08/05/2026 and was administered to the child that same day.

LPA reviewed the Parent Consent for Administration of Medications and Medication Chart (LIC 9221). The LIC 9221 reflected a beginning medication date of 08/05/2026; however, the form was not signed and dated until 08/07/2026.
