32 | However, interviews and documentation show that these protocols were not followed for the reported incident. The child reported their injury to staff, yet staff did not conduct an assessment for injuries, and no documentation was provided showing that the parent was notified. Additionally, the facility staff was not aware of the severity of the incident until later when the child's authorized representative was present for pick up. Interviews also indicate that staff did not seek any medical services, despite the fact that the child ultimately required medical care based on provided documentation.
Regarding operating beyond the scope of the license, the June 2025 waiver allows preschool and school-age children to use the same outdoor playground with the clear limitation that younger preschool children may not use the play structure. Interviews revealed that this stipulation was not followed. Preschool children were allowed onto the play structure, which is outside the conditions of the approved waiver and constitutes operating beyond the scope of the license. This allegation is therefore substantiated.
Based on observations, interviews, and documentation reviewed, the preponderance of evidence standard has been met; therefore, the allegations are SUBSTANTIATED. A Type B deficiency is cited on the attached LIC 9099D.
An exit interview was conducted and the report was reviewed with Hiedi Fields. A Notice of Site Visit was provided and must remain posted for 30 days. Appeal rights were provided.
