Licensing Program Analyst (LPA) D. Sanchez, made an unannounced Case Management inspection to the facility to conduct a site inspection. LPA met with Damaris Lopez, and tour the facility. LPA advised Damaris the purpose of today’s visit, to inspect the facility to ensure that the facility is not operating. During today's inspection, there were no children present in the facility. Ms. Lopez, stated that last day they provided care and supervision to daycare children was on May 31, 2024. As of June 1, 2024, they no longer operate the daycare. Ms. Lopez stated that her mom Maria Sanchez was not at home.

An exit interview was conducted with Damaris Lopez and a copy of this report left at the facility.
