Licensing Program Analyst (LPA) D. Sanchez, made an unannounced follow up Case Management inspection to the facility today in response to an Unusual Incident/Injury Reports received in the San Diego Child Care Regional Office (SDCCRO) on 5/31/2022, 6/15/2022 and 6/22/2022. The 5/31/2022, report states that one daycare child was terminated due to behavior issues. The 6/15/2022, report states that 911 was called to daycare due to teenage child having a psychiatric episode and the 6/22/2022, report states that 911 was called due to provider being in pain and taken to the hospital.

LPA interviewed provider and reviewed incident reports during today's inspection.

An exit interview was conducted with Elsa Beileson and a copy of this report left at the facility.

LPA observed provider placing the Notice of Cite Visit on the wall visible to parents during today’s inspection.
