On 1/25/23, A Case Management -Legal inspection was conducted by Regional Manager (RM) Adriana Hernandez and Licensing Program Analyst (LPAs) Elvin Baddley and Francisca Velazquez. The nature of the inspection was discussed with Licensee and Lenthonio Stevenson-Ramirez, to serve the following letters regarding the immediate exclusion of Lenthonio Stevenson-Ramirez:

ORDER TO INDIVIDUAL OF IMMEDIATE EXCLUSION FROM ALL FACILITIES
ORDER TO LICENSEE/FACILITY OF IMMEDIATE EXCLUSION FROM ALL FACILITIES

RM Hernandez advised Licensee and Lenthonio, that Lenthonio Stevenson-Ramirez is excluded from Ramirez Family Child Care Home and any other licensed facilities by the Department of Social Services. Appeal process and instructions have been provided.
