During the course of the investigation, LPA conducted interviews, received and reviewed pertinent documentation, and conducted observations to help determine a conclusion to the investigation. Based on interviews and observations, LPA determined that there is insufficient evidence to prove that a staff left child unattended on playground. Per interviews, the facility follows the name to face transition policy and there is a staff in the front and one in the back counting the children and repeating the procedure after each door that is passed.
Although the above allegation may have happened or is valid, based on LPA's observations, interviews, and record review which were conducted, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
No deficiencies were issued during today’s visit. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were also provided.
Exit interview conducted and report was reviewed with director, Chen “Zoe” Liang. |
