FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There is a working telephone at the facility. Disinfectants, cleaning products and medicines were stored inaccessible to the children in care in child locked or high cabinets. Furniture and equipment were age appropriate and in good condition, free of sharp, loose, or pointed parts. Floors in the facility are clean and safe. LPA observed solid waste containers with tight-fitting lids in the rooms. LPA observed appropriate changing tables in the restrooms and trash cans with tight fitting cover for the disposal of diapers. Napping equipment is appropriate. LPA inspected the napping equipment and observed them to be stored in a healthful manner. Restrooms and sinks for children were observed to be safe, sanitary, and in working condition. Urinals in the preschool bathroom were observed to be covered. Director and Licensee representative were informed to make those accessible as they were counted during capacity determination for the facility.

LPA observed serviced fire extinguishers, smoke and carbon monoxide detectors, and fire pull stations. The facility has a built-in smoke detector/ fire system. Log shows that the last fire Drill was conducted on 09/25/2026 which is compliant with the six-month requirement for facilities.

Drinking water is readily available for children in each room and in the outdoor playground area via individual water bottles and refilled using the water dispenser in Room 3. LPA observed water lead testing was completed by the facility prior to licensure on 01/11/2025 and an Action Level Exceedance was observed for the one faucet tested. Licensee Representative stated that the faucet was replaced and retested prior to licensure and will email the report of the retest to LPA by 10/09/2026. Licensee Representative stated that no faucets at the facility are used for drinking or food preparation purposes. The water dispenser is refilled using store bought water which is used to refill children's water cups.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information (https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information)

Director provided LPA with a copy of the water lead report showing the exceedance, External Water Sampler Self Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276) and facility sketch indicating the outlets tested during today's inspection.

Parents provide lunch and snacks for the children. Parents have the option to purchase lunch as well. LPA observed a monthly menu. Facility provides dry snacks to the children. Food and beverages were stored safely to prevent contamination and spoilage. Food storage and preparation areas were observed to be sanitary. All food and beverages that require refrigeration are stored in covered containers.

Outdoor playground and play equipment were inspected. LPA observed play equipment was in good condition and age-appropriate. The playground area is surrounded by appropriate fencing and the outdoor surfaces are safe. Shade is provided by canopies and building overhang. There is a climbing structure in the playground with artificial grass as resilient material. As the age recommendation on the play structure is for children ages 2-12 years, Director and Licensee representative were informed to follow the age recommended by the manufacturer and prevent access to children under 2 years of age. No bodies of water were observed.

Children sign in and out procedures and logs were reviewed. Two children were not observed to be signed in today and two were signed in on the wrong date. Children's roster was reviewed, and a copy was provided to LPA. LPA reviewed a sample of five children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. Needs & Services Plans and 15-minute nap checks and logs are not being maintained for children under 24 months of age.

LPA discussed the safe sleep regulations with Licensee Representative and Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee Representative and Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Two staff files were reviewed. A review of staff records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The name of the child care center director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department.
