32 | On 6/22/26 and 8/4/26 LPA Tammy Dutra and LPA Laird conducted interviews with two staff (S1-S2). All staff stated they knew about the incident involving C1 hurting their mouth. Both staff stated they did not observe how C1 hurt their mouth. Both staff stated they were in the classroom when the incident occurred and were actively supervising children. S1 stated they were standing close to C1 when the injury occurred but they did not see how C1 hurt their mouth.
Based on interviews, LPA Laird determined there is insufficient evidence at this time to determine if the injury to C1 occurred due to lack of supervision. Although staff were working within ratio and capacity requirements and both staff were in the classroom, neither staff saw how C1 injured their mouth.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated.
Exit interview conducted and report was reviewed with the facility director, Jessica Amaya. Appeal rights were provided.
