An informal meeting was conducted with licensee, Cherilynn Aase on 9/18/2023 at 1:00pm, via teleconference. The meeting was attended by Licensing Program Manager, Erin Virrueta and Licensing Program Analyst, Nicolette Cunningham.

The meeting was held to discuss the following deficiency.


On 9/6/23, the licensee was cited a type A citation for lack of supervision for an incident that occurred on 8/31/23. Child 1 (approximately 2 years old) was found on the side of O’Conner Avenue (approximately 50-60 yards from the facility) by a stranger that was driving by (Witness 1). The licensee admitted that Child 1 exited the facility through the front door without her knowledge at approximately 9:00am on 8/31/23. The licensee stated she was supervising a group of six children in the living room and her assistant was supervising a group of five children in the daycare room. The licensee stated she walked away from the group to move laundry into the dryer, heard the doorbell ring and found Witness 1 and Witness 2 at the door with Child 1. The licensee estimates the child was unsupervised for approximately 5-6 minutes.

LPM Virrueta reviewed Care and Supervision regulations with licensee, Aase. The licensee stated she has added two additional baby gates, created a new procedure to conduct head counts, and completed training on active supervision. The licensee stated she has reviewed the new procedures with her two assistants.

The facility will be placed on increased monitoring for the next two years beginning today, 9/18/2023. The licensee understands that continued noncompliance may lead to administrative action.
