13 | This is an amended report. On 11/8/23 at approximately 3:30pm, Licensing Program Analyst (LPA) N. Cunningham conducted a closing complaint investigation inspection and met with the licensee. It was alleged that the provider does not ensure the day care home is kept clean. The Licensee was interviewed 08/16/23 and denied the allegation and stated she always maintains a clean and sanitary home. Two staff were interviewed on 10/9/23 and 10/18/23, both staff acknowledged they frequently clean while providing care. Four parents (P3, P4, P9, P10) were interviewed on 10/18/23. P3 and P4 reported a strong smell of urine in the home. P9 reported the home is not clean and has a strong small of urine. P10 stated they recalled an odor at the home. On 11/8/23, LPA observed a strong smell of urine in the home. LPA also observed the home unkept and cluttered. Based on the parent statements and LPA’s observation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. Exit interview conducted and report was reviewed with the licensee. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
