On March 7, 2024 at approximately 10:15am, an unannounced complaint investigation visit was made to the facility by Licensing Program Analyst (LPA) N. Cunningham. It has been alleged that the facility is malodorous; specifically, there is a strong odor of cat and dog urine in the facility which is causing an uncomfortable environment for children. It has also been alleged that the home is unsanitary; specifically,there's dog feces all over the house. It was also reported that the home is cluttered and unorganized. At approximately 10:30am, the licensee stated she recently had a small baby in care that took a lot of her time so she was unable to let her dog outside to go to the bathroom. At approximately 10:41am, LPA observed and photographed dog feces in the carpet in one bedroom where a daycare child was resting. The licensee stated she knew the hallway smalled like urine and will shampoo the carpet tonight. LPA observed a stong smell of urine in the playroom by the door leading to the bathroom and in the hallway by the bathroom. LPA photographed the carpet in the areas the smell was observed. LPA observed the home to be cluttered and unkept. At approximately 11:56am, LPA observed Child 1 sitting on the floor close to the area with the strong urine smell pick up a toy and place it in their mouth.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. California Code of Regulations, Title 22 is cited on the attached LIC 9099D. Exit interview was conducted, notice of site visit posted, and appeal rights were provided.

LPA N. Cunningham informed licensee, Sara Harris, that this report dated 3/7/24 documents 2 Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.


Also, LPA N. Cunningham informed the licensee to provide a copy of this licensing report dated 3/7/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.


Notice of Site Visit shall be posted for 30 days from today's visit.

The licensee stated she will shampoo the carpet tonight. The licensee will rearrange the kitchen and remove items. The licensee will organize and deep clean her home this weekend. The licensee stated she is in the process of hiring two assistants. The licensee stated she will look for a housekeeper. The licensee will e-mail LPA and provide an update on 3/15/24.
