25 | A Case Management inspection was conducted at the facility by Licensing Program Analyst (LPA) Y. Yang in response to an Unusual Incident Report (UIR) received on 08/12/2026 regarding a child sustaining an injury while in care. During today’s visit, the LPA met with the facility’s Center Director, Anais Heim, to discuss the reported incident and obtain additional information.
The facility reported that on 08/05/2026, child C1 was climbing on a free standing, dramatic play wardrobe located in the classroom. At the time, C1’s teacher, Staff S1, was at the changing table assisting another child with a diaper change. The facility reported that S1 was facing C1 and observed C1 climbing on the wardrobe. S1 reportedly attempted to verbally redirect C1 multiple times and instructed C1 to get down. The facility reported that after S1 completed the diaper change, she proceeded toward C1 to assist the child in placing their feet back on the floor. As S1 approached C1, the dramatic play wardrobe reportedly tipped over and landed on C1’s wrist.
The facility reported that S1 immediately assisted C1 and provided ice and comfort. S1 observed a red mark and slight swelling on C1’s wrist. S1 then notified the facility’s Assistant Director, Addie Ligmanowski (Staff S2). The facility reported that S2 examined C1 and observed that C1 was not crying, was able to freely move the wrist, and did not display any apparent signs of pain or discomfort. The facility reported that the Assistant Director subsequently contacted C1’s authorized representative and notified them of the incident. C1’s authorized representative arrived at the facility approximately one hour later, examined C1, and spoke with C1’s teacher regarding the incident. C1 was then taken home for the remainder of the day and reportedly returned to the facility the following day without any apparent issues.
The facility reported that C1 was evaluated by a medical professional on 08/06/2026. The medical evaluation determined that C1 had sustained a buckle fracture to the right arm and would need to wear a brace on the wrist.
During today’s Case Management inspection, the LPA reviewed the facility’s child supervision policies with the Director. The LPA also inspected the classroom environment and observed that the dramatic play wardrobe involved in the incident was age-appropriate for the children in care. The Director stated that the wardrobe has since been moved and will be securely anchored to a wall as a precaution. The facility reported the incident to the California Department of Social Services, Community Care Licensing Division (CCLD), and notified the child’s authorized representative as required by applicable regulations.
Based on available information, it has been determined that there was no evidence of violations of regulations that occurred that lead to the child's injury. This report was read and reviewed with the Center Director, Anais Heim. There were no Title 22 violations cited during today's case management visit. Appela Rights provided. The Notice of Site Visit shall be posted for 30 days.
