*****This is an Amended document from visit of 8/02/24.******
During the initial Complaint-Investigation to the facility on 04/08/24, and a follow up Complaint-Investigation to the facility on 6/28/24, records were reviewed, and the LPA conducted interviews with the Licensee. The Licensee stated denied the allegations and stated, "I always provide a safe and clean environment for day care children, I do a daily cleaning after the day care children leave and make sure there are no broken object around the entire day care, nor outside". Licensee stated that my staff and I provide adequate supervision to day care children, we never leave them alone. Licensee stated no day care child has sustained unexplained injuries while in my care. I always notify the parents if something happens to their child while in my care. Licensee stated if I notice a bruise or any marks on children when they arrive my day care, I always ask parent what happened to their child. Licensee stated, "I have never handled a day care child in a rough manner". Licensee stated at the moment only one child uses a pacifier and only uses it's own, I never give children pacifiers that doesn't belong to them. Licensee stated a previous child (C1) used a pacifier, but C1 had an allergic to the skin, but not caused by the pacifier.
During the investigation, the LPA made observations, and obtained additional information. An interview was conducted with Staff 1 (S1) and Child 4 (C4). During the LPA’s inspections to the facility, the LPA did not observe any evidence of personal rights violations, or any children being yelled at by a staff member or mistreated.

Based on available information, although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations did or not occur, therefore, the allegations are determined to be Unsubstantiated at this time.
