Licensing Program Analyst (LPA), Amy Strother made a subsequent complaint investigation inspection, for the purpose of delivering complaint findings, and met with facility representative, Monika Desor (S3). The following has been alleged, 1. Staff not properly cleaning daycare child during diaper changes, specifically that the week of 05/11/26 a child came home with a poorly cleaned diaper area including some fecal residue.

During the initial investigation inspection on 06/22/26 LPA Strother toured the facility, received a current roster of children in care, reviewed records and conducted an interview with staff (S3).

S3 stated that on one occasion it was brought to her attention by a parent that their child was not thoroughly cleaned during a diaper change and that it only happened once. S3 stated that she didn’t have any concerns about diapering, but brought the parents concern to the staff person she thought changed the diaper.

During the investigation LPA Strother interviewed six adults, (A1-A6) and three former staff (S2, S4 & S5) between 06/15/26 and 09/02/26, with three attempts made to conduct an interview with S1 between 09/04/26 and 09/09/26.

All six adults shared experiences of observing that their child was not thoroughly wiped while at the facility. Four out of 6 adults stated that their child had fecal matter on their skin from not being thoroughly wiped during a diaper change while in care at the facility. One adult stated that their child was newly potty trained when enrolled but felt that their child was not getting support in getting fully wiped. The adult stated that this was despite Desor stating that the facility offered potty training and the adult informing Desor at the time of enrollment that their child still needed assistance, resulting in rashes developing.

Only one staff corroborated that a couple of parents had complained that their child was not thoroughly cleaned during a diaper change at the facility.

On 06/22/26 LPA Strother received an email with documentation of a text message sent between a parent (a different parent from the one S3 mentioned during the 06/22/26 interview with LPA) and Desor (S3). The text message was sent to Desor on 02/27/26 stating that when their child got home from the facility there was “poop” that wasn’t wiped off still on the child’s buttock. Desor replied, stating that she would ask staff about it on Monday, apologizing for the incident and thanking the parent for letting her know.

Based interviews conducted and a review of records received, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22 are cited on the attached LIC9099-D.

This report was reviewed and discussed with Facility Representative, Monika Desor. Appeal Rights were provided.

Notice of Site Visit shall be posted for 30 days from today's visit.
