On 09/23/2026, Licensing Program Analyst (LPA) Pa Kou Vue conducted an unannounced inspection to deliver findings for a complaint report received by the department on 07/24/2026. LPA met with Director Katie Bonzi and explained the purpose of the inspection. LPA toured the facility and took a census.

During the course of investigation, LPA conducted facility observations, conducted interviews, obtained and reviewed pertinent documents pertaining to the above allegation. Based on LPA observations conducted on 07/28/2026, 08/04/2026 and 09/23/2026 and interviews, observations and interviews cannot determined if staff did not provide adequate supervision resulting in a child causing harm to other children in care. Although the above allegation may have happened or are valid, there is not a preponderance of evidence at this time to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
