25 | On 09/17/2026, Licensing Program Analyst (LPA) Anita Tristan conducted an unannounced case management visit regarding an unusual incident reported to the Department on 08/26/2026. LPA met with ECE Lead, Margaret Mclain and explained the reason of the visit. The incident occurred on 08/25/2026 at approximately 1:20 PM and involved Child #1 getting injured on a play structure.
According to staff, Child #1 was playing on the play structure with a slide. Staff had instructed Child #1 several times to sit on their bottom when sliding. Child #1 slid down on their knees, attempted to jump off the slide, and fell, landing on their right arm. Staff administered first aid and escorted Child #1 to the school nurse. The parent was notified, and Child #1 was taken to Urgent Care, where the child was confirmed to have a fractured right arm.
At the time of inspection, Child #1 had returned to the center. The teacher who was present during the incident was out sick during today’s visit.
During the inspection, LPA reviewed files, interviewed staff, and took photographs. This appears to be an isolated incident. Staff responded appropriately, followed proper procedures in addressing the injury, and met reporting requirements.
LPA advised staff that best practice would be to redirect a child from the play structure if they are not following safety rules and there is an imminent concern for injury.
Per Title 22, Division 12 of California Code of Regulations, no deficiencies are being cited. Appeal Rights were provided and discussed.
An exit interview was conducted with Margaret McLain. A Notice of Site Visit was issued and must remain posted for 30 days.
