On August 27, 2026, Licensing Program Analysts (LPAs) Xona Xayavong and Valentin Hernandez conducted an unannounced complaint inspection. LPAs toured the inside and outside of the facility and a census was taken. LPAs met with Site Supervisor Megan Doyel. LPAs explained the purpose of the inspection was to deliver findings regarding allegations received by the Department.

During the course of the investigation, LPA Xayavong conducted three unannounced complaint inspections, interviewed staff, parents, and children, reviewed facility records and documentation, and observed the facility environment.

Allegation: Staff did not meet a child’s diapering care needs.
During interviews, parents expressed positive feedback regarding diapering practices, and staff confirmed that periodic diaper checks are conducted throughout the day. LPA reviewed diaper logs documenting multiple entries throughout the day for children. (Continue LIC 9099-C)

LPA was unable to obtain sufficient evidence to determine that staff failed to meet the child’s diapering care needs.

Although the concern may have occurred or could be valid, a preponderance of evidence does not exist to prove the alleged violation did or did not occur. The allegation is UNSUBSTANTIATED.

Allegation: Staff did not provide sufficient drinking water to a child in care.
Interviews and observations confirmed that the facility keeps multiple gallons of water available, including refrigerated water for immediate use. Staff reported that water is offered frequently throughout the day, especially during outdoor activities. LPA observed a substantial supply of drinking water stored on site. LPA was unable to obtain sufficient evidence to determine that staff failed to provide adequate drinking water.

Although the concern may have occurred or could be valid, a preponderance of evidence does not exist to prove the alleged violation did or did not occur. The allegation is UNSUBSTANTIATED.

Allegation: Staff did not follow a child’s modified diet.
Interviews with parents confirmed that the facility provides alternative meals and snacks to accommodate dietary restrictions. LPA reviewed menus and documentation showing modified dietary plans for children requiring allergen specific diets and verified this information in children’s files. LPA also observed posted documentation in the kitchen noting children with modified diets. LPA was unable to obtain sufficient evidence to determine that staff failed to follow the child’s dietary accommodations.

Although the concern may have occurred or could be valid, a preponderance of evidence does not exist to prove the alleged violation did or did not occur. The allegation is UNSUBSTANTIATED.

Allegation: Staff exposed a child to the sun for an extended period of time.
Staff stated that children are not allowed outdoors when temperatures exceed 90 degrees. Parents shared that they are informed of the facility’s heat safety practices and encouraged to provide hats and sunscreen. LPA was unable to obtain sufficient evidence to determine that the child was exposed to the sun for an extended period. (Continue on LIC 9099-C)
