How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

CCR 101223(a)(3) — Personal Rights. To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion,…

“Record review documented that Staff (S1) physically handled infants in a rough and punitive manner during aggressive time-out placements. Staff (S1) and Staff (S2) failed to console or comfort crying infants, demonstrating a failure to meet infants’ basic care needs. Additionally, S1 placed infants on prolonged time-outs by confining them in highchair-style equipment and similar furniture as a form of discipline or punishment. Although no visible injuries were observed, this conduct posed an immediate risk to infants’ personal rights, health, and well-being.”
