Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“facility out of ratio. The licensee failed to ensure proper infant - teacher ratio to provide safe environment. This poses an immediate risk to the health, safety, or personal rights of children in care. Witnesses, Staff #1 & Staff #2 (S1 & S2), revealed that while supervision was in placed, Child #1 (C1) sustained a head injury when he fell backwards from a sitting position (the incident occurred in the playground) due to teacher - child ratio (3 teachers to infants).”
“interviews of witnesses and review of records. The licensee failed to ensure proper staff to infant ratio to provide care and supervision. This poses an immediate risk to the health, safety, or personal rights of children in care. Witnesses revealed that infant rooms #1 & #2 were out of ratio with infants and two teachers on Picture Day, May 2023. Records review corroborate witnesses’ statements”
“LPA visual observation of infant chairs with jagged edges. The licensee failed to ensure furniture are safe and in good repair at all times. This poses a possible risk to the health and safety of children in care.”
“LPA visual observation of chairs utilized by infants but not age appropriate for infants under months old per manufacturer’s product specification. The licensee failed to ensure furniture are safe and age appropriate. This poses a possible risk to the health and safety of children in care.”
