Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on statements and records obtained which confirmed the Licensee was absent from the facility more than 80% of the time during her daily hours of operation.”
“Based on LPA observation, there was a child (C1) who was sleeping in a stroller in the living room which poses an immediate risk to the health, safety, and personal rights of persons in care.”
“Based on Licensee submitted fraudulent documents to receive Subsidy Program funds by showing inaccurate time in and out entries on monthly records at her facility, while Licensee was out of the Country, and children were found not to be in attendance, which poses an immediate health and safety risk to the children in care.”
