Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“.. Based on documentation reviewed and interviews conducted, the Department determined that the facility failed to follow its established policies and procedures regarding the disenrollment of Child (C1) by never completing the family pledge/ engagment packet.”
“.... Based on two of two interviews and records reviewed, C1 was left alone at the end of a hallway for approximately five or more minutes. C1 was located when appropriate name-to-face documenation was reviewed. Which poses a potential health, safety and/or personal rights risk to children in care.”
“Based on records review, C1 was served food that C1 was allergic to which poses a potential health, safety and/or personal rights risk to children in care.”
“LPA observed childen in Classrom #2 with one staff member and children inside classroom #3 with two staff members. This poses an immediate risk to the health and safety to children in care.”
“A child got his/her head stuck in a gap between the wall and the sink inside classroom #2 for approximately five minutes. This is a potential risk to the health and safety of children in care.”
