Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“LPA observed Child #1 (C1) napping in play pen with a blanket inside the playpen and a sheet hanging on the side of the play pen.”
“Based on interviews/record review, A1 provided transportation for the child care on 9.18.24, and additional unknown dates. A1 did not have a California clearance when transportation was provided. This poses an immediate Health, Safety, Personal Rights risk to persons in care. Civil penalties issued.”
“Based on interviews, and record review, the licensee did not ensure, C1, C2 and C3 were provided a safe and comfortable environment when C1, C2 and C3 were left unsupervised on September 18, 2024. This poses an immediate Health, Safety, or Personal Rights risk to persons in care.”
“Based on interviews, care and supervision was provided to C1 in the licensee’s home for periods of more than hours. This poses a potential Health, Safety, or Personal Rights risk to persons in care.”
“Based on interviews, the licensee did not report two incidents that were unusual and threatened the emotional health or safety of the children in care. This poses a potential Health, Safety, or Personal Rights risk to persons in care.”
“Based on interviews and record review C2 was enrolled on September 17, 2024, and did not have the required immunizations prior to enrollment. This poses a potential Health, Safety, or Personal Rights risk to persons in care.”
