Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“, Based on LPAs observation in Purple Room 2, children were left unsupervised while C1 was in restroom and S1 left room to assist C1 in restroom. This is a potential health and safety risk to children in care.”
“interviews and video footage. Based on interviews and a review of video footage, S1 admitted to grabbing C1’s chin and speaking to them in a harsh tone. S1 grabbed C1 on their lap and did not allow the C1 to get up when they attempted to do so. This poses a potential health and safety risk to the children in care.”
“Based on interviews, on 11/2/2022, due to lack of communication and/or training, staff did not timely change a child's diaper prior to pick up from the facility, resulting in leakage and wet clothing. This is a potential risk to the health and safety of children in care.”
“Based on LPA's interview with the director and observation of children indoors who were not wearing face coverings and Licensee not enforcing the use of face masks/coverings of children. This is an immediate hazard to the health of children due to the COVID-19 pandemic which is currently on the rise.”
